At Elite Care Homes Ltd we are dedicated to delivering Person Centered Care, all our staff are trained to recognise the importance of this. By adopting a Person Centered Care approach the service we offer are far more flexible and open.
CHOICE
We achieve this by giving each resident an individual care plan, which is drawn up after consultation with the resident and other family members or advisors. our care plans fully take into account the residents likes and dislikes.This results in every resident receiving care specifically designed to meet his or her needs.
Our Care plans in our care homes are totally Person Centered managed, by looking at what the residents wants or needs, and what they don’t want or don’t need based on their unique life history, personality and preferences expressed.
Our staff regularly review each individual care plan, to make sure needs continue to be met. Residents and their family are involved as far as is practical in the decisions regarding their care.
Person Centered Care also means that we are able to encourage residents to decide how they want to live during their stay with us. Residents can decide when they want to get up and go to bed, which activities they want to take part in and what they would like to eat.
Every resident in our care homes is assigned a care worker who works with the resident. Our staff focus is on respecting the preferences of residents, and treating them as partners when setting goals, planning care and making decisions about care, treatment or outcomes.
Before admission it is advisable to come and visit our homes and staff, to satisfy yourself and see if the ambience is to your liking.
